Calcasieu Parish School System's Policy on Immunization Requirements
Your child's immunizations must be up-to-date to enroll in the school system.
State law requires a school booster of DPT, Polio and a
second MMR. The DPT and Polio must be given on or after the 4th birthday and prior to
entrance to school. The second MMR can be given a minimum of 30 days after the first MMR,
if necessary to meet the entrance requirements.
The minimum required standard for school entry is:
- four (4) DPT's with one given after the
child's 4th birthday
- three (3) Polio's with one given after the
child's 4th birthday

Hepatitis B required for kindergarten
entry. This is a series of three (3) injections. This requirement is also for

Varivax (Chicken pox vaccine) Two
(2) injections or a history of the disease
required for children entering kindergarten.
